ACC services we deliver

You don’t need to select a service when referring. The list below provides an overview of the ACC programmes we coordinate.

Integrated Care Pathway

Coordination model for complex injuries. ICP is how Habit Health brings together the right services for people with complex needs through a single referral. You don’t need to request ICP specifically.

Pain Management Service (PMS)

Interdisciplinary support for persistent and complex pain.

Concussion Services

Assessment and rehabilitation for mild to moderate traumatic brain injury.

Physiotherapy (EPN)

Acute and ongoing physical injury rehabilitation.

Hand Therapy

Specialised physical therapy for increasing functioning of the upper extremity including the shoulder, elbow, forearm, wrist and hands.

Vocational Rehabilitation Services

Stay at Work (SAW), Back to Work (BTW) and Job Support Services (JSS)

Psychological Services

Injury-related psychological assessment and treatment.

Social Rehabilitation Assessment (SRNA)

Identify home-based care needs and equipment that will assist a client to return to independence in the home.

Nursing Services

Injury-related community nursing support.

Assessments

Functional Capacity Evaluations (FCE), Neuropsychological and Occupational Assessments.

Sensitive Claims Services

Trauma-informed psychological care.

Living My Life (LML)

Support for people with significant injury-related disability.

Equipment, Wheelchair & Housing Support

Assessment and coordination of equipment and modifications.

Training for Independence (TI)

Supports people to build the practical skills and confidence needed to live as independently as possible following injury.

Educational Based Rehabilitation Assessment (EBRA)

Specialist assessment that evaluates how a child’s injury affects learning and participation at school and identifies the supports needed in the classroom.

Nationwide delivery, local co-ordination

Addressing health concerns early helps prevent issues affecting work performance, attendance and long-term wellbeing.

Services coordinated across North and South Island regions.
Local, in-person delivery where available.
Telehealth options used where clinically appropriate.

No. Send one referral and our team will triage and place the person into the most appropriate programme.

Who can refer to ACC programmes?

Referrals are commonly made by health professionals, ACC, insurers and authorised parties.

Do I need to request ICP specifically

No. You do not need to request ICP. Send one ACC referral and our team will determine whether the Integrated Care Pathway is appropriate.

Are services available nationwide?

Yes. Habit Health delivers services across New Zealand with local and telehealth options.

How quickly are clients contacted?

Once a referral is received and confirmed, we make contact within 1 business day.
